Uncontested Child Custody/Visitation

Includes payment of the filing fee and the drafting of an Original Notice, Petition to Establish Paternity, Custody, Visitation and Support, Affidavit of Financial Status, Child Support Guidelines Worksheet, Children in the Middle Cover Page, Stipulation and Agreement, Decree Establishing Paternity, Custody, Visitation and Support, and an Income Withholding Order. Uncontested Divorce with children

Original Notice and Petition

Includes the drafting of an Original Notice and a Petition for Dissolution of Marriage, Petition to Establish Paternity, Custody, Visitation and Support, Petition to Terminate Parental Rights, Petition for Adoption or Petition for Name Change customized to meet your needs. Filing fees and service fees not included.

Affidavit of Financial Status

Includes the drafting of an Affidavit of Financial Status where client provides a fully completed financial questionnaire.

Child Support Guidelines Worksheet

Includes the drafting of one Child Support Guidelines Worksheet.

Stipulation and Agreement

Includes the drafting of a Stipulation and Agreement outlining the parties’ agreement on the division of assets and debts, alimony, legal and physical custody, visitation, child and medical support, child tax credits and other similarly related topics. If drafted for a divorce case without children or a child custody case, subject to flat fee. If drafted in a divorce case with children (involving both the division of assets/debts and custody/visitation/support), subject to flat fee.

Decree of Dissolution of Marriage

Includes the drafting of a final Decree of Dissolution of Marriage, which can include a name change back to a maiden surname.

Uncontested Step-Parent Adoption

Includes the drafting of an Original Notice, Combined Petition to Terminate Parental Rights and to Adopt, Consent to Termination of Parental Rights, Consent to Adoption, Order Appointing GAL, and Decree Terminating Parental Rights and Granting Adoption. Flat Rate is for 1 child. Each child must have a separate case. Additional children can be added for a fee per child. Service fees not included.

Terminating support in a corollary case can be included for an additional fee.

A name change, which involves the completion of forms required by the Bureau of Vital Statistics by the State that issued the birth certificate, can be included for an additional fee. Any processing fees charged by the State not included.

Uncontested Name Change Action

Includes payment of the filing fee and the drafting of an Original Notice, Petition for Name Change, Consent, and Decree of Name Change. Service fees not included.

The completion of forms required by the Bureau of Vital Statistics by the State that issued the birth certificate, can be included for an additional fee. Any processing fees charged by the State not included.

Qualified Domestic Relations OrdeR

Includes the drafting of a Qualified Domestic Relations Order and any supporting forms required by the plan administrator. Client must provide a Summary Plan Description, Plan Administrator’s QDRO Procedures, and any Sample QDRO to Attorney. Certain types of plans excluded from flat fee.

Military Pension Division Order

Includes the drafting of a Military Pension Division Order and DFAS Form 2656-1. Client must obtain and provide all military forms needed to draft Order.

Quit Claim Deed

Includes the deed necessary to transfer ownership and interest in real estate. Recording fees apply if a Client desires to have Attorney record the executed deed with the Recorder’s Office. Recording fees include a flat fee plus fee per each additional page (based on the number of pages in the final deed) and transfer fee. An electronic filing fee may also apply.

Income Withholding Order 

Includes the drafting of the court order used to garnish the payment of child support and spousal support from a payor’s work wages.
